Q. Are silicone gel breast implants safe?

Q. Is it better to place the implant above or below the muscle?
A. Placement above the muscle had a much higher association rate with capsular contracture, as well as rippling and sagging, known as "the baseball phenomenon." It is also much more difficult to perform a proper mammogram due to the interference of the breast implant next to the breast tissue. Implants placed under the muscle gives a much more accurate mammogram since the muscle is in between the implant and the breast tissue. Placement of the implant below the muscle allows it to be supported by the muscle and not the skin tissue, making the ripples virtually invisible and the implant much less palpable. This gives a much softer and smoother look and can increase the longevity of the implant to 15 - 20 years, as opposed to much less if placed above the muscle. Of note, 40% of surgeries that I perform are above to below conversion operations from patients operated on by other physicians.
Q. Is it true that placing implants below the muscle is a more painful procedure?
A. There is slightly more discomfort when the implant is placed below the muscle. But modern pharmacology can minimize the pain. The added advantage is that the procedure will last ten to twenty times longer than the above muscle placement - it is worth the two-day discomfort for proper positioning. It would be very shortsighted to undergo a procedure that has a slightly easier postoperative result, but significantly less satisfying long-term results.
Q. What is the recovery period for augmentation mammoplasty?
A. My patients typically get back to work within 3 to 4 days, with only mild discomfort requiring, at the most, Tylenol for pain. We qualify the results as looking good at one month and looking great after four months. This time period is required for the body to adapt, the skin and muscle tissue to stretch and the implant to gain its final soft appearance.
Q. Can I breast feed after augmentation mammoplasty?
A. There should be no interference whatsoever with breast feeding after submuscular placement of the implants. I have had several patients who have had at least one child after surgery and have breast fed with no problems. And none of these patients required further surgery. However, if the implant is placed above the muscle this may interfere with lactation.
